The Council produces the electronic directories described below. We update the information annually. Information contained in the directories may not be used for the purpose of solicitation or direct mail.

HRSA Health Care For The Homeless Grantees Directory

The HRSA Health Care for the Homeless Grantees Directory provides information on grantees of the Federal Health Care for the Homeless Program- also known as Section 330(h) of the Public Health Service Act-and their subcontractors. The intent of the directory is to foster collaboration and communication among these HCH projects, and provide a starting point for finding health care resources for homeless people.

2011-2012 Health Care For The Homeless Clinicians’ Network Directory

The National Council provides the HCH Clinicians’ Network Directory solely as a service to Network members to facilitate networking by state, clinical discipline or topics of clinical interest. The Network membership represents the various clinical disciplines working in homeless health care including nurses, social workers, physicians, outreach workers, advanced practice nurses, case managers, oral health providers, and more. This online directory is not a complete listing of members or member contact information; members may elect not to be included. The directory is a compilation work and owned solely by the Council; use of the Network membership directory for commercial purposes is strictly prohibited.

Medical Respite Programs In The United States And Canada

The Medical Respite Care Program Directory contains descriptions of known respite care programs in the U.S. and Canada. These programs provide safe shelter, medical and nursing care and aftercare planning that homeless people need to recover from illnesses. Services are provided wherever beds are available in the program’s community, and locations include transitional housing programs, nursing homes and assisted living facilities, motels homeless shelters, substance use treatment centers, apartment unites and stand alone respite care facilities.

Agencies that share the National Council’s mission may apply for Organizational Membership; members include grantees and subcontractors of the Federal Health Care for the Homeless funding stream, other health care providers, and advocacy organizations.
