TennCare Shelter Enrollment Project Data Report Form
Each quarter TennCare Shelter Enrollment Project participants report the number of children who have engaged in their program each month, how many were insured, what kind of insurance they have, whether or not they were able to enroll in TennCare if needed, and any medical care they may have accessed before exiting the program. The project uses this data to determine the success of TennCare among children who are without stable housing.
Our goal is for all children living in Tennessee shelters to be enrolled in and enjoying access to TennCare and consistent, quality medical care.
