According to the National Committee for Quality Assurance (the most common accrediting agency for health centers), the “patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.” Today, a majority of Health Care for the Homeless health centers have at least one site accredited/recognized for PCMH, most by NCQA, the Joint Commission, or the Accreditation Association for the Ambulatory Health Care (AAAHC). Most of the following resources were produced when PCMH was still a burgeoning initiative for HCH health centers, yet we retain them here for any useful information that remains.
