Standard clinical guidelines often fail to take into consideration the unique challenges presented by homelessness that may limit an individual’s access to needed services or ability to adhere to a plan of care. To fill this gap, the HCH Clinicians’ Network developed a series of adapted clinical guidelines covering the following:
