Consumer Subsidy Information

Applications are due August 23, 2019.  Late or incomplete applications will not be considered.

 

Eligible Applicants

The National Health Care for the Homeless Council will provide a limited number of subsidies to support consumer participation in the 2019 MAT&R Training in Portland, OR. Consumer subsidies will be offered only to HCH grantees to support attendance of consumers who:

  • Have experienced homelessness and;
  • Received services from the HCH grantee and;
  • Are members of the National Consumer Advisory Board (join for free at nhchc.org).

Subsidy Benefits

The National Health Care for the Homeless Council will:

  • Pay the cost of training registration ($200).
  • Provide recipients with a cash per diem ($149) to cover meals not included in the conference and incidental expenses (the per diem will be distributed Monday morning at registration).
  • For attendees outside of the Portland area, we will provide two nights of lodging (Sunday and Monday nights) and $200 for travel costs.

The HCH representative will:

  • Register subsidy recipient for the training via the online form using the fee waiver code by September 3, 2019 (waiver code will be emailed by the National HCH Council staff on August 26, 2019).
  • Make and pay for subsidy recipient’s travel reservations.
  • Submit the reimbursement claim form and supporting documentation no later than September 27, 2019, the Council will only reimburse for $200 of the travel costs. The reimbursement form will be emailed by Council staff with the subsidy notification. No reimbursements will be paid for training “no-shows.”

To be considered, the application must be received no later than August 23, 2019.

Decisions regarding consumer subsidy applications will be communicated to applicants and consumers via email no later than August 26, 2019.
